INFORMED CONSENT, LIABILITY WAIVER & SERVICE AGREEMENT

Welcome to Syrene Therapies & Consulting!

This Agreement ("Agreement") is entered into by and between the undersigned participant ("Client") and Syrene Therapies & Consulting, LLC, including but not limited to its affiliates, subsidiaries, agents, contractors, therapists, practitioners, instructors, employees, and any collaborative partners operating under the Syrene brand ("Syrene").

I. ACKNOWLEDGMENT OF SERVICES

By booking and paying for services with Syrene Therapies & Consulting LLC, I, the Client, voluntarily consent to receive one or more of the following services offered by Syrene Therapies & Consulting:

Neurosomatic therapy, touch therapy, Thai/Swedish massage, Ra Sekhi (Kemetic Reiki), chiropractic inspired techniques, and all forms of bodywork and healing sessions.

1. Dance therapy, acrobatic movement, kundalini movement, somatic therapy, yoga, and breathwork.

2. Group workshops, transformational programs, and wellness retreats.

3. Mobile sessions rendered at my home, workplace, temporary residence, hotel, vehicle, or outdoor/ remote setting.

4. Participation in sessions at collaborative venues, partner spaces, and third-party properties.

5. Interaction with and usage of wearable technology, including future implementation of haptic suits, virtual reality (VR), artificial intelligence (AI) systems, biometric feedback systems, and other digital innovations.

II. CONSENT TO PHYSICAL TOUCH & SOMATIC ENGAGEMENT

I understand and consent to the use of intentional, professional, and therapeutic touch during my sessions with Syrene Therapies. I affirm that:

• All touch is consensual, respectful, and within the boundaries of wellness-based therapeutic practice.

• I may withdraw or modify my consent to touch at any time.

• I understand that emotional and physical responses may arise as part of the healing process and agree to communicate any discomfort or boundaries clearly.

III. ASSUMPTION OF RISK & RELEASE OF LIABILITY

I acknowledge that the services rendered by Syrene are alternative, complementary, and holistic in nature and are not substitutes for medical diagnosis or treatment. I fully understand that:

• There are inherent risks associated with any bodywork, movement, or wellness practice, including but not limited to muscle soreness, dizziness, emotional release, or temporary discomfort.

• The use of experimental technologies such as VR, AI, or haptic suits involves evolving tools which may have unforeseen outcomes or adjustments.

Accordingly, I release, waive, and forever discharge Syrene, its owners, agents, collaborators, and practitioners from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me during participation in the aforementioned services.

IV. MOBILE AND OFFSITE SESSIONS

I understand that:

• Mobile therapy sessions are conducted in diverse locations that may or may not be under the direct control of Syrene.

• I am responsible for ensuring a safe, private, and suitable space for therapy when receiving mobile services.

• Syrene assumes no responsibility for injury or damages that occur due to environmental conditions outside of its control.

If I decline use of the therapist’s professional equipment (such as massage table, linens, or coverings) and request services to be performed on my own surfaces (e.g., bed, couch, or furniture), I accept full responsibility for any staining, wear, or damage to my property. Protective coverings will be used when possible, but they do not guarantee prevention of oil transfer, stains, or damage.

V. CONFIDENTIALITY & MEDIA RELEASE

All personal information shared with Syrene is kept strictly confidential, in accordance with ethical and legal standards. I grant or deny (circle one) Syrene permission to record, photograph, or document sessions, workshops, or testimonials for marketing and educational purposes. I understand that my identity will remain protected unless I give explicit written permission.

VI. PAYMENT, CANCELLATION & REFUNDS

I agree to pay all fees for services rendered, whether prepaid or due post-session. I understand that cancellations made less than 24 hours in advance are non-refundable. I acknowledge that deposit or retainer fees are non-refundable and serve to secure the therapist’s time and energy.

• Once a session has begun, no refunds or partial refunds will be issued under any circumstances, including but not limited to dissatisfaction with results, property issues, or environmental conditions.

• Any claim regarding damage must be submitted in writing with photographic evidence and a valid receipt for cleaning or replacement. Syrene reserves the right to review and deny claims that do not meet these standards.

• Payment for services constitutes full agreement to all terms, policies, and waiver provisions, whether or not I physically sign this document. Proceeding with services without a physical signature constitutes implied consent.

VII. GOVERNING LAW & DISPUTE RESOLUTION

This Agreement shall be governed by the laws of the State of Texas. Any disputes arising from this Agreement shall first be subject to informal negotiation, and if unresolved, to mediation in Travis County, Texas before resorting to litigation.

VIII. ENTIRE AGREEMENT & SEVERABILITY

This document constitutes the entire agreement between the Client and Syrene. If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

BY MAKING PAYMENT FOR SERVICES, I ACKNOWLEDGE THAT I HAVE READ, UNDERSTOOD, AND AGREE TO THE TERMS AND CONDITIONS OUTLINED ABOVE. I FREELY AND VOLUNTARILY ACCEPT ALL RISKS INVOLVED.
